About The Questions

There are several packages to choose from based on your and your family's needs. I want to create menus that  work for your lifestyle, and for you and your family members. This survey is just a beginning at determining how I can create tasty healthful meals that you and your family will enjoy eating. We can work out something just for you. Nature's Chef Questionnaire

 

This is a preliminary "taste and lifestyle" form you can fill out. There are more questions that the Chef will ask before your first appointment. These are the questions that the Chef will use to make preliminary menu suggestions for you.

  2. Do you have any medical conditions that affect your diet? (For Example are you a diabetic? Do you have ulcers? Are you on a cholesterol lowering diet. Are you on any medication that does not allow you to eat certain food products. Are you on any diet prescribed by a doctor?)

     


  3. Do you have any food allergies or sensitivities that might affect your food choices?


  4. Are you a vegetarian or a vegan? Any other diet that you follow: macrobiotic, ovo-lacto vegetarian, Mc Dougal Vegetarian, raw food diet, etc?


  5. Do you abstain from eating any foods due philosophical reasons. If so what kind?


  6. Are you avoiding, but not abstaining totally from: meats in general, seafood, beef, pork, soy based products, fatty foods, wheat products, shellfish, carbohydrate intensive foods, sugars, processed foods, cooked foods?


  7. Do you prefer your foods to be organic or as free from pesticides, growth hormones and additives as possible?


  8. Is it worth it to you to pay as much as twice as much for clean organic foods?


  9. Do you have a favorite ethnic or style of cooking?


  10. What restaurants do you frequent?


  11. Do you like exotic foods or unusual things? Or do you prefer familiar comfortable items?


  12. Do you prefer a predictable diet composed of the same or similar items or to you like a lot of variation in your meals?


  13. Do you like traditional meals of an entrée, vegetable and starch or do you prefer to graze on smaller portions of different things?


  14. Do you want breakfasts? Lunches? Or primarily Dinners?


  15. Is there anything you simply can’t stand? Is there any food, or type of food that you really liked as a child?


  16. Do you like large or small portions in general?


  17. Do you eat a lot of snacks? If so what?


  18. What about desserts and sweets?


  19. What are your favorite foods?

     

  20. Is there anything you could eat every day and not get tired of?


  21. Do you have any family or other traditions you like to observe (like fish on Friday, Pasta on Wednesdays)?


  22. Do you plan a weekly menu or decide what to have on a daily basis. Would you like to have it more planned?


  23. Is there anyone else who will be eating that has different or maybe opposite preferences than yours that we need to consider?


  24. Will you be doing any holiday entertaining that we might help you with?
